The following must be submitted in order to facilitate the evaluation/processing of claim and/or issuance of our Letter of Authority to Repair (LOA).



CHECKLIST OF CLAIMS REQUIREMENTS


OWN DAMAGE (Damage to your vehicle)

- Police Report
- Driver’s Statement/Affidavit
- Photocopy of Driver’s License with Official Receipt
- Photocopy of Registration Certificate with Official Receipt
- Picture of damaged portion
- Photocopy of Policy
- Photocopy of Official Receipt of Premium Payment
- Stencil of Motor & Chassis numbers
- Repair Estimate

THIRD PARTY PROPERTY DAMAGE

- Police Report
- Affidavit/Statement of both drivers
- Driver’s License with Official Receipt of both drivers
- Photocopy of Registration Certificate with Official Receipt both vehicles
- Pictures of damaged portion of both vehicles
- Photocopy of the Policy of the Insured with Official Receipt
- Stencil of Motor & Chassis numbers of both vehicles
- Repair Estimate
- Certificate of No Claim from the Insurer of the adverse party

THIRD PARTY BODILY INJURY

- Police Report
- Driver’s Statement/Affidavit
- Driver’s License with Official Receipt
- Registration Certificate with Official Receipt
- Photocopy of Official Receipt of Premium payment
- Photocopy of Certificate of Cover or Policy
- Original Medical Certificate
- Original Medical Receipt
- Stencil of Motor and Serial Numbers
- Hospital Statement of Account
- Original Prescription Receipt

DEATH CLAIM

- Police Report
- Driver’s Statement/Affidavit
- Driver’s License with Official Receipt
- Registration Certificate with Official Receipt
- Photocopy of Certificate of Cover
- Photocopy of Official Receipt Premium payment
- Certified True Copy of Death Certificate of Victim
- Certified True Copy of Birth Certificate of Victim
- Certified True Copy of Marriage License of Parents of Victim
- Original Funeral Receipts

- Prepare a copy of your Policy.

- Submit the required documents.


For assistance, you may call our Stronghold Insurance Company, Inc. Claims Hotline at (632)891-1329 to 37 immediately or the branch office near you to report the incident.



- Report the accident to the nearest Police for assistance and secure a copy of the Police Report;

- Get the name and address of the driver and owner of the adverse vehicle (if possible);

- Get the name of the Insurance Company of the adverse vehicle (if possible);

- Report the accident to our office nearest your place and secure the claims documents.
